*Preregistration for New Enrollees*
(Child does not need to attend preregistration.)

If your child will be 3, 4, or 5 years old by Sept. 1, 2010, please

bring the following:

• Shot record
• Birth Certificate (large document from Department of Vital Statistics, not small card)
• Social Security Card
• Two Proofs of Residency (one from each category):

1. Tax receipt, mortgage statement, or rental/lease agreement

2.  Cable bill, electric/gas bill, telephone bill (not cell phone bill), water/sewer bill

You will be given an appointment at that time to bring your child back for screening which will be held:

Friday, March 12,  8:00 a.m.-2:00 p.m.

Please note that students who are presently enrolled at FMPS or SPS  do not need to attend preregistration or screening.

Optional full-day preschool for 3 year olds!
